Register Now!

All parents/guardians are required to complete and submit the following:

  • Completed Registration Form
  • After-Care Registration (if applicable)
  • Health History/Medical Form with current immunization records
  • Student Conduct**
  • Field Trip Authorization**
  • Authorized Pick-Up Form****Student Conduct, Field Trip Authorization and Authorized Pick-Up Forms will be available in-person and at the parent meeting.

If the above documents are not submitted prior to first day of the program, students will not be allowed to start. Please make every effort to complete all documents and pay appropriate tuition and fees.

Register by TBD to Avoid a $10 Late Registration Fee. Register Early… Register Now – Limited enrollment! Register by phone, fax, mail or in-person Monday – Friday, 9am – 4:30pm OR Saturday, June 8th from 9am-1pm in Room 3198 (Green Area)

Mail-in: Mail check or money order and completed forms to Essex County College. When register-ing by mail, your check will serve as your re-ceipt. Call department to confirm if received.

24-Hour Fax Registration: Complete the registration form and include AMEX, Discover, MasterCard, or VISA infor-mation. Call department to confirm if received.

After-care Registration Form »
This form can be completed in advance or in-person. In-person, your child needs to be signed-up and payment made the Wednesday prior to the week he/she will be attending.

Health History/ Medical Form »
This form must be completed, listing all allergies, signed (by parent), and returned with a CURRENT copy of the child’s immunization record from their Physician. NOTE: If a child needs medications by injections or an allergy kit, the child or Parent/guardian must administer that medication. In addition, if an asthma inhaler needs to be used, the participant must be able to self-administer and it must be labeled with his/her name and kept with the participant during program hours.

Student Conduct Agreement (provided in person)
Upon registration, this form will be given. It should be reviewed by the parent with each participant enrolling in the program. This is an agreement between participant, parent and department which states appropriate/acceptable behavior for each participant to follow during the duration of the program. All participants and parents must read and sign this agreement and return prior to start of their program.

Field Trip Authorization (provided in-person)
Will be provided during the first week of the program to be returned by the specified date.
